## Which program fits you?
The questions that genuinely come up when choosing between Core, Evolve and Ultimate. General questions on process, reimbursement and preparation are in our FAQ.

Which program is right for me? + That depends less on budget than on the question you want answered. Core is right if you want to know where you stand for the first time: 25 functional examinations and 87 laboratory biomarkers in one day, after which you know which two or three topics matter now. Evolve is right if you also need the structural picture, meaning whole-body MRI and liquid biopsy, plus guidance across the year. Ultimate is for people who want year-round management with a dedicated health manager. If you are unsure, take the smaller program: you can add to it later, but you cannot undo a package that was too large.

Do I need the whole-body MRI? + This question drives the largest price difference, because MRI is the main addition from Core to Evolve. There is no universal answer. For whole-body imaging in people without symptoms there is no evidence of a mortality reduction, and it regularly produces incidental findings: in the literature around one in three people scanned, of which the smaller share is serious. It becomes worthwhile above all when there is a family history or a concrete suspicion. Without such a reason, the most likely outcome is a work-up chain. In the free intro call we will also tell you when Core is enough in your situation.

When is Ultimate not worth it? + If what you mainly expect is more numbers. Ultimate adds epigenetics, neurology and microbiome analysis to Evolve, and a considerable part of that sits explicitly at our research-adjacent evidence tier. Those procedures are documented and medically interpreted, but on their own they trigger no treatment. So anyone hoping for a treatment decision out of those values is paying for something they do not deliver. What genuinely sets Ultimate apart is the year-round support with a dedicated health manager. If you do not want that, or would not use it, Evolve is the more honest choice. Which procedure sits at which evidence tier is set out in full on our evidence page.

How does the time commitment differ between the programs? + Core takes around 6 hours, Evolve and Ultimate around 9 hours each. The difference comes mostly down to whole-body MRI, which alone accounts for about 60 minutes and is not included in Core. There is no time difference between Evolve and Ultimate, because Ultimate’s additional scope sits mostly in the laboratory rather than on the examination day. In every program, all imaging and functional procedures sit on a single day, as does the first medical results consultation in the afternoon. Genetics, liquid biopsy and specialist laboratory work take longer: a second consultation follows for those, usually after two to three weeks. So plan for two appointments, not one.

What justifies the price jump from Core to Evolve? + Core costs 1,900 euros, Evolve 7,600. Four items make the difference: 60 minutes of whole-body MRI on a 3 Tesla scanner, the liquid biopsy with its laboratory costs, the step from 87 to 120 laboratory biomarkers, and support across the year. The honest part: whole-body MRI and liquid biopsy sit at our evidence tier 3. For people without symptoms their benefit is not established, and a finding from them triggers no treatment here. So the premium buys scanner time, laboratory costs and physician time, not more established evidence. The guideline-based part is in Core: blood pressure, ApoB, Lp(a), HbA1c, cardiopulmonary exercise testing, cardiac and vascular ultrasound. If you want to know where you stand for the first time, take Core. Evolve is right if you explicitly want the structural picture and accept the uncertainty that comes with it.

What is included in the price and what is not? + Included are all examinations in the respective program, the 60-minute medical results consultation on the examination day, the written documentation and the prevention plan. Evolve and Ultimate add support across the year; Core does not. Not included is anything that only follows from a finding: specialist work-up, further imaging or treatment. Those steps belong in standard care, which is where we hand over. On billing: we invoice under the German medical fee schedule, and people with private insurance may be reimbursed in part depending on their policy. Exactly what gets reimbursed depends on the contract and cannot be promised in general beforehand.
