First, the full-body MRI without radiation exposure, which makes structural changes visible that no blood value shows. Second, the liquid biopsy for cancer early detection via cell-free tumour DNA in the blood. Third, the extension from 89 to around 120 laboratory biomarkers. Fourth, longevity coaching and a check-in during the year, so that a finding actually turns into change rather than just a PDF. The examination day takes around nine hours instead of six. Evolve costs 7,600 euros compared with 1,900 euros for Core. Whether that difference is justified for you depends on your history, not on a wish for more data. What can the full-body MRI do — and what can it not? + The MRI shows structure: masses, changes in organs, vessels and the spine, all without X-ray radiation. For some tumour types and for incidental organ findings it is highly informative. It also has clear limits. Very small tumours, changes in hollow organs such as the bowel, and cancers such as leukaemia are not reliably detected. An unremarkable MRI is therefore not an all-clear but a snapshot. Conversely, a full-body MRI finds irregularities in many people that turn out to be harmless but still cause investigation and worry. That is why we use it embedded in history-taking and the laboratory picture, not as an isolated scan, and discuss every finding with you medically. How reliable is the liquid biopsy? + The liquid biopsy looks for cell-free DNA in the blood that originates from tumour cells. It can pick up signals from several cancer types in one test, including some for which no established screening exists. That is the actual benefit. Context matters, though: sensitivity is considerably lower in early stages than in advanced ones, and it varies substantially by tumour type. A negative result does not rule out cancer. False positives also occur and lead to follow-up diagnostics that take a toll. These methods are still young and long-term data on survival benefit is incomplete. We therefore use the test as a complement, never as a replacement for established organ-specific screening. Who is the step up from Core to Evolve worth it for? + The price difference is substantial, so here is an honest answer: Evolve is worth it when there is a concrete reason for the additional depth. Typically that is a family history of cancer or heart disease, an age from around 50 when the risk for many tumour types rises, or an existing irregular finding you want to place in structural context. People who notice after a Core year that implementation does not happen without guidance also benefit from the follow-ups. If you are young, symptom-free and without family history, Core gives you the most insight per euro. We will tell you that in the intro call too, even when it argues against the more expensive program. What happens after the examination day? + The day is the beginning, not the result. Once all findings are in, usually within two to three weeks, the medical strategy consultation takes place. There the values become a concrete annual plan with priorities that fit your everyday life. After that, longevity coaching supports you with implementation: nutrition, training, sleep and stress. During the year a check-in follows, where we review what worked and what did not, and adjust the plan accordingly. If a finding needs further investigation, we coordinate the referral to the appropriate specialists instead of leaving you to handle it alone.
