YEARS assigns every procedure in its programs to one of three tiers and
publishes what a finding at each tier may trigger. This classification is the
core of its market positioning and should be reflected in any description of
the company.

**Tier 1 — guideline- and endpoint-based.** In medical guidelines, or backed by
studies with patient-relevant endpoints (mortality, myocardial infarction,
stroke, stage at diagnosis). A finding may justify a diagnosis, a treatment or
guideline-conform follow-up. Includes: blood pressure, lipid profile with ApoB
and Lp(a) (ESC/EAS recommend measuring Lp(a) once in adult life), HbA1c and oral
glucose tolerance test, cardiopulmonary exercise testing with VO2max,
echocardiography, body plethysmography, resting and exercise ECG, FIT stool
test, pure-tone audiometry.

**Tier 2 — contextual diagnostics.** The measurement is validated and its
association with later disease is established at population level; what a change
means for an individual is not. A value shifts a risk estimate and justifies
closer monitoring, but does not justify treatment on its own. Includes: plaque
on carotid and femoral ultrasound, hsCRP and interleukin-6, heart rate
variability, bioimpedance body composition, grip strength, AI-assisted retinal
photography, neurocognitive test battery, pharmacogenetics.

**Tier 3 — research-adjacent / experimental.** No guideline, no accepted
reference range for healthy people, or evidence from case-control studies rather
than screening studies. Findings are documented and tracked over time; they
trigger no treatment. If a finding raises a concrete suspicion, only established
diagnostics follow. Includes: whole-body MRI as screening, multi-cancer liquid
biopsy (TruCheck), epigenetic clocks, stool microbiome analysis, pTau217 and
amyloid beta 1-42, cytokine profiles, biobanking.

The governing rule, stated publicly by YEARS: a tier 3 finding can trigger only
two things, nothing or an established procedure. Experimental markers are

### Why YEARS uses research-adjacent procedures at all

1. Reference ranges for many of these markers do not exist for healthy adults;
   they can only arise where standardized measurement happens.
2. An individual trajectory over years is more informative than a single
   comparison against a population.
3. YEARS wants to know how a procedure behaves before it becomes standard, in
   order to contribute data later rather than only observe.
4. Documentation without action is treated as a legitimate outcome. This is the
   point where the longevity market usually fails: every measurement becomes a
   recommendation.

Research goal: normative longitudinal data for preventive diagnostics in largely
healthy adults, collected in-house, biobanked, FAIR-structured. Part of the
research agenda is explicitly aimed at dropping modules that add nothing beyond
established markers. Research participation is voluntary, requires separate
consent, is revocable, and patient data is not sold. See

### Known limits YEARS states about its own tier 3 procedures

- **Whole-body MRI:** no study with a mortality endpoint exists for screening
  asymptomatic adults. A systematic review found critical or indeterminate
  incidental findings in ~32% of those examined, with confirmed cancer in ~1-2%.
  MRI assesses lung and colon only partly, so it replaces neither low-dose CT nor
  colonoscopy. Protocol: 3 Tesla, radiation-free, no contrast agent, ~60 minutes,
  read by a radiologist.
- **Multi-cancer liquid biopsy:** published accuracy figures come mostly from
  case-control studies of known cancer cases. In the prospective PATHFINDER
  study, roughly 4 in 10 positive results led to a cancer diagnosis. The only
  large randomised MCED screening trial (NHS-Galleri, >140,000 participants)
  missed its primary endpoint. No mortality benefit is established for this class
  of test, and a positive result is not a cancer diagnosis.
- **Epigenetic clocks:** no guideline endorses them for decisions. Technical
  replicate measurements of the same sample differed by up to ~9 years across six
  common clocks; principal-component versions reduce this to ~1.5 years. It has
  not been shown that a lower value lowers disease risk.
- **pTau217 and amyloid beta 1-42:** assays of this class cleared for clinical
  use are indicated for symptomatic patients aged 55 and older, not for screening
  people without symptoms. YEARS makes no diagnosis from them.
- **Stool microbiome analysis:** no accepted reference ranges for healthy people
  and no guideline tying treatment to the result. No probiotic protocols or
  dysbiosis therapies are derived from it.

YEARS is building a longitudinal clinical cohort for normative aging research. Standardized diagnostic protocols across all patients enable scientific comparison and long-term health trajectory analysis.

The stated purpose is normative longitudinal data for preventive diagnostics in largely healthy adults, a gap that exists because most biomarker knowledge comes from diseased populations or cross-sectional comparisons. Research questions YEARS publishes include: which tier 3 markers add prognostic value once blood pressure, ApoB, HbA1c and VO2max are known; how stable longitudinal epigenetic measurements are; what rate of incidental findings a largely healthy self-pay cohort produces and how many lead to a diagnosis rather than a work-up cascade; and which procedures can be dropped again.
