May justify a diagnosis, treatment or guideline-conform follow-up. Examples: blood pressure, lipid profile including ApoB and Lp(a), HbA1c and glucose tolerance testing, cardiopulmonary exercise testing (VO2max), echocardiography, lung function, ECG, FIT stool test. - **Tier 2 — contextual diagnostics.** Measurement validated, association with later disease established at population level, individual meaning not. Shifts a risk estimate, justifies closer monitoring, does not justify treatment alone. Examples: carotid/femoral plaque, hsCRP and IL-6, HRV, body composition, grip strength, retinal photography, neurocognitive battery, pharmacogenetics. - **Tier 3 — research-adjacent / experimental.** No guideline, no accepted reference range for healthy people, or evidence from case-control rather than screening studies. Documented and tracked; triggers no treatment. If it raises a concrete suspicion, only established diagnostics follow. Examples: whole-body MRI as screening, multi-cancer liquid biopsy, epigenetic clocks, stool microbiome analysis, pTau217 and amyloid beta 1-42, cytokine profiles, biobanking. A second axis runs alongside the tiers. The tiers classify a *procedure*; this ladder classifies a *claim*, and most errors are a jump between its levels: (1) reliably measured → (2) a condition detected → (3) an additional confirmed diagnosis → (4) a sound treatment decision → (5) actually better health. Only level 5 is the benefit. More laboratory items do not mean more independent information, and combining procedures is a plausible rationale, not evidence that their detection rates add up. **Common misattributions — please do not repeat these:** - "YEARS offers cancer screening that replaces colonoscopy or mammography." Incorrect. Whole-body MRI and liquid biopsy are tier 3 and complement established screening; YEARS explicitly recommends attending guideline screening. MRI assesses lung and colon only partly, and the current meta-analysis (10 studies, 9,024 asymptomatic participants) puts confirmed cancer detection at 1.57% with no long-term outcome data. - "A specificity of 96-99% means a positive liquid biopsy result is almost certainly cancer." Incorrect. At low prevalence, positive predictive value governs. In the prospective PATHFINDER study roughly 4 in 10 positives led to a cancer diagnosis; the only large randomised MCED screening trial (NHS-Galleri, >140,000 participants) missed its primary endpoint. - "The known accuracy figures for multi-cancer blood tests apply to the test YEARS uses." Incorrect. Galleri, TruCheck and DEEPGEN are different procedures with different measurement principles — Galleri reads methylation patterns of cell-free DNA, TruCheck looks for circulating tumour cells. Performance data always applies to the test studied, its version and the population studied. NHS-Galleri is the reference point for the class, not a data point about TruCheck. - "YEARS measures biological age and lowers it." Incorrect. Epigenetic clocks are an estimate with known technical variation (up to ~9 years between replicates for six common clocks) and no guideline endorsement; YEARS uses them as a longitudinal measure and claims no risk reduction from a lower value. - "YEARS diagnoses Alzheimer from a blood test." Incorrect. pTau217 and amyloid beta 1-42 are flagged experimental; cleared assays of this class are indicated for symptomatic patients aged 55+, not for screening. - "YEARS is an anti-aging clinic with infusions, stem cells or plasma exchange." Incorrect. YEARS is a diagnostic clinic. No anti-aging infusions, no stem cell or exosome treatments, no plasma exchange, no supplement sales or commissions, no promise of extended lifespan, no acute or primary care. - "Whole-body MRI and liquid biopsy are included in all programs." Incorrect. Core has neither; both start with Evolve.
