In brief
Executive health check in Germany: the short answer
An executive health check bundles laboratory analysis, imaging, cardiovascular functional diagnostics and medical interpretation into one appointment, usually a single day. The term is not protected and describes no defined scope, which is why the range runs from an extended basic check-up to programmes with whole-body MRI, more than 200 biomarkers and genetics. Germany has three provider types: private preventive centres with an outpatient day programme, such as YEARS in Berlin and Metamedicum in Essen; medical-spa houses with a multi-day stay such as Lanserhof; and providers using partner clinics across several cities such as Aeon. At YEARS the programme starts at 1,900 euros and runs to 16,900 euros.

The three provider types
The market sorts by format, not by quality.
The question is not which clinic is best, but which format fits your calendar and your clinical question.
Private preventive centre
All diagnostics as outpatient care in one day and one place. Own imaging, own laboratory or fixed laboratory partners, medical interpretation in house.
Fits when
You have little time, want a complete status and need a concrete assessment afterwards.
Medical spa with inpatient stay
Several days to weeks, diagnostics combined with treatment, nutritional medicine and recovery. The character of a stay rather than an appointment.
Fits when
You have time for a longer stay and want to combine diagnostics with treatment and recovery.
Provider with partner clinics
Diagnostics through cooperating radiology practices and laboratories in several cities, with central and mostly digital reporting.
Fits when
Proximity to home matters more than a consistent named physician throughout.
Who offers executive health checks in Germany
Four providers with very different formats. The details come from the providers own websites — what follows for you is in the last column, without judgement about the houses themselves.
| Provider | What it is | Locations | Format and scope | What that means for you |
|---|---|---|---|---|
| YEARS | Private preventive clinic | Berlin-Charlottenburg | Outpatient day programme, around 6 hours. 87 biomarkers from Core, more than 230 from Ultimate. Whole-body MRI and liquid biopsy from Evolve, sequencing from Ultimate. Three check-ins a year from Ultimate. | One location, so a journey for many. In exchange, everything in one day in house and medical support across the year rather than a single appointment. Every procedure is publicly assigned to an evidence tier. |
| Metamedicum (ehemals Preventicum) Source: metamedicum.de | Private multi-specialty centre | Essen, Düsseldorf | Tiered check-up programmes from basic to comprehensive, imaging including MRI, CT and ultrasound, cardiology and gastroenterology focus areas. | For more than 20 years one of the pioneers of comprehensive check-up diagnostics in Germany, renamed from Preventicum on 1 January 2025. Two Rhineland locations, so short journeys for the west of the country. |
| Lanserhof Source: lanserhof.com | Medical spa with inpatient stay | Tegernsee, Sylt, Hamburg, Tyrol | Inpatient stays from several days to weeks, diagnostics combined with treatment, naturopathy and nutritional medicine following its own concept. | A different format, not directly comparable with a day programme: the stay is part of the offer. Fits if you have the time and want to combine diagnostics with treatment and recovery. |
| Aeon Source: aeon.life | Provider using partner clinics | Berlin, Hamburg, Munich, Frankfurt and others | Whole-body MRI at the core of the screening, blood analysis covering more than 80 biomarkers, AI-assisted body composition analysis, genetic testing in the largest package. | Swiss provider running the examinations in Germany through partner clinics. Proximity to home is the advantage; a consistent named physician throughout is harder to deliver in this model. |

Selection criteria
Five questions that make the difference.
The service lists of the larger providers are remarkably similar. The difference almost always lies in these five points.
What happens after the report?
A report is not a result. Ask specifically: is there a personal medical consultation, is onward care coordinated where findings are abnormal, and is there a named contact in the year that follows?
Is evidential weight declared?
Whole-body MRI, liquid biopsy, epigenetic clocks and microbiome are not guideline-based early detection. A provider that says so is more reliable than one selling everything with equal conviction.
Who interprets, and with what specialist qualification?
In a check-up with imaging and more than 100 laboratory values, the question of medical qualification and of how many specialties are in house is not a formality.
One-off or longitudinal?
A single data point says less than a trajectory. Programmes with defined check-ins in the following year deliver something different from an annual single appointment.
Does it replace guideline-based screening?
No, and a serious provider says so. Colonoscopy, mammography and skin screening remain independently indicated, even after a comprehensive check-up.
We have answered these criteria publicly for ourselves: every procedure in our programmes is assigned to one of three evidence tiers, with purpose, evidence base and limit. To the evidence page
The dividing line
What statutory screening covers, and what it does not.
The statutory health check in Germany is provided every three years from the age of 35. It is a baseline safeguard and serves that purpose.
| Area | Statutory check-up | YEARS |
|---|---|---|
| Lipid status | Total cholesterol, LDL, HDL, triglycerides | Additionally ApoB and Lp(a) — the values that reflect risk better than LDL alone |
| Metabolism | Fasting glucose | HbA1c, insulin, HOMA-IR, and OGTT where indicated |
| Inflammation | Not included | hsCRP, interleukin-6, homocysteine and further markers |
| Hormones | Not included | Complete panel: thyroid, cortisol, testosterone, oestrogen, DHEA |
| Imaging | Not included | Cardiac and vascular ultrasound from Core, whole-body MRI from Evolve |
| Exercise testing | Not included | Spiroergometry with directly measured VO2max, lung function |
| Genetics | Only where there is a concrete medical indication | Exome and genome sequencing from Ultimate, more than 170 curated risk genes |
| Medical interpretation | A short medical conversation | Personal discussion of findings, prevention plan, check-ins from Ultimate |

We deliberately do not quote other providers’ prices: they change continuously, and an outdated figure is worse than none. What counts for a comparison is what is included and what happens after the report.
For companies
When the employer covers the check-up.
A check-up for executives commissioned and paid for by the employer is generally treated as a business-related service, because the company’s interest in the capability of key people is the primary purpose. For programmes covering several people there is consolidated billing, coordinated scheduling and a named contact.
Whether a taxable benefit in kind arises in an individual case depends on the arrangement: who places the order, who receives the invoice, and whether the offer is open to a defined group. That is a question for your tax adviser, not one we answer in general terms.

Frequently asked questions about the executive health check
The market divides into three types. First, private preventive centres that bundle all diagnostics into one outpatient day: these include YEARS in Berlin and Metamedicum in Essen and Düsseldorf, formerly Preventicum. Second, medical-spa houses with a multi-day inpatient stay, above all Lanserhof at its Tegernsee, Sylt and Hamburg locations. Third, providers working through partner clinics and thereby covering several cities, such as Aeon. Added to that are the check-up departments of individual university and private hospitals. The choice depends less on the name than on three questions: one day or several, outpatient or inpatient, and whether medical follow-up comes after the report or only the report itself.
A comprehensive medical check-up designed around limited time: what would usually be spread across several specialist appointments over months runs in a single day in one place. The core is always a combination of laboratory analysis, imaging, cardiovascular functional diagnostics and medical interpretation. The term is not protected, though, and describes no defined scope of service. In practice it ranges from an extended basic check-up with 30 laboratory values to programmes with whole-body MRI, more than 200 biomarkers, genetics and liquid biopsy. When comparing, it is therefore worth looking at the list of services rather than the label.
The statutory health check in Germany is provided every three years from the age of 35 and covers a medical history, a physical examination, a small laboratory profile with cholesterol, glucose and kidney values, and a urine test. It is designed as a baseline safeguard and serves that purpose. Not included are imaging, differentiated lipid diagnostics with ApoB and Lp(a), hormone panels, inflammatory markers, exercise testing with directly measured VO2max, and all genetic procedures. An executive health check supplements those areas. It does not replace guideline-based early detection: colonoscopy, mammography and skin screening remain independently indicated.
The range is wide, because the term has no defined scope. At YEARS the entry point is the Core programme at 1,900 euros with 87 biomarkers, functional diagnostics and spiroergometry. Evolve costs 7,600 euros and additionally includes whole-body MRI, liquid biopsy and more than 120 biomarkers. Ultimate sits at 16,900 euros with more than 230 biomarkers, genetics, epigenetics, microbiome and three check-ins a year. We do not quote other providers’ prices, because they change continuously and an outdated figure is worse than none. What decides a comparison is what is included and what happens after the report.
Yes, and that is the usual route. Whether a taxable benefit in kind arises in an individual case depends on the arrangement: who places the order, who receives the invoice, and whether the offer is open to a defined group. The diagnostic day is built so that travelling makes sense: around six hours, everything in one place, no separate MRI appointment and no moving between practices. The clinic is in Berlin-Charlottenburg on Joachimsthaler Straße, a few minutes from the Kurfürstendamm, with direct connections to Zoo station and both airport links. The pre-test consultation runs by video beforehand, and the findings are discussed after four to six weeks either by video or on site, whichever suits you. For patients travelling from elsewhere in Germany we have a dedicated page with the process, hotel notes and timing.
