At 40 your body changes quietly — and on several fronts at once.

Three biological processes start to become measurable in your forties. Identify them early and you have 30+ years to influence them.

Cardiovascular plaque

ApoB particles and Lp(a) have been depositing in your vessels since your thirties. By 40, early calcification is often already detectable — long before any symptom. Measuring early can often reverse what later can only be slowed.

Hormonal change

Oestrogen, testosterone, DHEA and thyroid values begin to shift. Symptoms such as loss of energy, poor sleep or muscle loss are rarely coincidental — they are often measurable hormone deficits.

Metabolic drift

Insulin resistance often begins 10-15 years before a diabetes diagnosis. The oral glucose tolerance test reveals early signs that standard fasting glucose misses.

What we examine especially thoroughly from 40 onwards.

More than 25 functional tests, 87 biomarkers and imaging — focused on the risks that start to surface in your forties.

ApoB & Lp(a)

The two most important cardiovascular risk markers — considerably more informative than LDL cholesterol alone. A standard check at a family doctor does not test for them.

Complete hormone profile

Oestrogen, testosterone, progesterone, DHEA-S, free T3/T4, diurnal cortisol profile. Detects subclinical deficits that affect energy and performance.

Detecting insulin resistance early

Oral glucose tolerance test with insulin measurement. Shows changes long before HbA1c becomes abnormal.

Whole-body MRI (from Evolve)

High-resolution imaging for early tumour indicators, aneurysms and organ changes — radiation-free, without contrast agent.

VO2max & spiroergometry

Your cardiorespiratory fitness is the strongest predictor of your life expectancy. We measure it precisely — and show where you stand for your age.

Neurocognitive baseline

Tests of memory and reaction performance create a reference value for the years ahead — important for early detection of dementia.

Frequently asked questions about the check-up from 40

Why is 40 the right age for a comprehensive check-up?+

From 40, cardiovascular risks (plaque formation), hormonal change and metabolic shifts begin — usually silently and without symptoms. Measuring early often lets you reverse with lifestyle changes what would later require medication.

What about the statutory check-up from 35?+

The statutory check from 35 covers only basic values (cholesterol, blood glucose, blood pressure). Important markers such as ApoB, Lp(a), hormone profile, insulin resistance, bone density and whole-body MRI are not included — precisely the values that become informative from 40 onwards.

Which programme makes most sense for someone aged 40?+

Core (€1,900) is the entry point with 87 biomarkers and functional tests. If you want the full picture once, including whole-body MRI and cancer liquid biopsy, choose Evolve (€7,600). Ultimate (€16,900) adds genetics and microbiome — sensible with a family history.

Will private health insurance reimburse the check-up?+

With many private insurers and policies, partial or full reimbursement is possible. We issue a detailed report with diagnostic codes that you can submit to your insurer. The exact reimbursement depends on your individual policy.

How long does the check-up take?+

A YEARS day takes 6 to 9 hours depending on the programme — everything in one day, in Berlin-Charlottenburg. Breakfast and lunch are included. You receive your detailed report within a week, often directly in a personal consultation on the same day.

How often should I repeat the check-up?+

From 40 we recommend an annual rhythm for the baseline values (Core), and Evolve or Ultimate every 2-3 years. That builds a longitudinal data set and lets you spot changes early.
