Iron Deficiency

Iron is relevant for hemoglobin production and numerous enzymatic processes, among other functions. The oxygen-carrying capacity of blood depends substantially on hemoglobin concentration.

Ferritin, by contrast, primarily serves as a marker of iron stores. Iron deficiency can already exist despite normal hemoglobin levels and may be associated with fatigue or reduced performance, particularly in endurance athletes.

Inflammation

High-sensitivity C-reactive protein (hs-CRP) is a marker of low-grade systemic inflammatory activity.

An elevated value is nonspecific and can have many causes. It does not automatically explain why VO₂max is low. In combination with other findings, however, hs-CRP can help identify inflammatory processes or increased cardiovascular risk.

Glucose Metabolism

Fasting glucose and HbA1c provide important information about glucose metabolism, but they do not detect every early abnormality.

Insulin measurements and derived values such as HOMA-IR can provide additional information about insulin sensitivity. However, HOMA-IR is not a universally standardized diagnostic threshold test for insulin resistance in the general population.

Thyroid Function

Overt hypothyroidism can cause fatigue, reduced exercise tolerance, and metabolic changes, among other symptoms.

TSH and free T4 are particularly important for basic assessment. Free T3 can be measured as an additional parameter in certain situations but is usually less important for the typical diagnosis of hypothyroidism. What Exactly Does a VO₂max Test Measure? Cardiopulmonary Exercise Testing Explained Simply

The term “VO₂max test” is often used imprecisely. In a medical setting, it usually refers to cardiopulmonary exercise testing, also known as CPET or spiroergometry.

It is considered the reference method for directly measuring cardiorespiratory fitness.

Definition of VO₂max

The value describes the maximum amount of oxygen your body can take up and use during very intense exercise.

Relative values are usually expressed as milliliters of oxygen per kilogram of body weight per minute: ml/kg/min.

When true maximal exertion is reached, the term VO₂max is often used. If no clear plateau in oxygen uptake is reached, VO₂peak is scientifically more precise.

How Cardiopulmonary Exercise Testing Works

The examination is performed, for example, on a bicycle ergometer or treadmill.

You wear a breathing mask that continuously measures oxygen uptake and carbon dioxide output. The workload is increased gradually or continuously until a high or maximal individual level of exertion is reached.

Depending on the setting and medical question, medical CPET may additionally monitor ECG, heart rate, and blood pressure.

More Than Just One Number

Professional cardiopulmonary exercise testing provides considerably more than maximum oxygen uptake.

Ventilatory thresholds: These help identify different exercise zones and metabolic transitions as intensity increases. For training prescription, they are often more useful than a generic percentage of maximum heart rate.

VE/VCO₂ slope: This parameter describes the relationship between ventilation and carbon dioxide output and provides information about ventilatory efficiency. In clinical cardiology and pulmonology, it can provide important diagnostic and prognostic information.

Heart-rate response and recovery: The heart-rate pattern during and after exercise provides additional information about fitness and autonomic regulation.

How It Differs From Other Methods

Exercise ECG

A conventional exercise ECG assesses heart rate, rhythm, blood-pressure response, and ECG changes during physical exertion, among other parameters. Without respiratory gas analysis, oxygen uptake is not measured directly.

Field Tests

Tests such as the Cooper test can estimate aerobic performance based on the distance covered. They depend more heavily on motivation, running economy, test conditions, and day-to-day form.

Wearables

Garmin, Apple Watch, WHOOP, and other wearables estimate VO₂max or cardiorespiratory fitness using heart rate, speed, power output, and additional sensor data.

These estimates can be useful for tracking trends. However, individual values may differ meaningfully from a direct respiratory gas measurement.

For medical assessment or precise performance diagnostics, direct cardiopulmonary exercise testing therefore remains the better method.

A good VO₂max value depends strongly on age, sex, body composition, training status, and the testing method used. A single universal threshold for “good” or “bad” is therefore of limited value. Reference values should be interpreted according to age and sex.

Which Blood Tests Usefully Complement a VO₂max Test?

A comprehensive blood panel can put the CPET results into context. The goal is less about measuring as many values as possible and more about identifying relevant potential influencing factors.

Iron Status: Oxygen Transport and Energy Metabolism

A complete blood count including hemoglobin is central.

Inflammatory Markers

hs-CRP can indicate low-grade systemic inflammatory activity.

The marker is nonspecific and should not be overinterpreted in isolation or immediately after very intense physical exercise.

Metabolism

Vitamin D should not be interpreted according to a universal “optimal threshold.” Its interpretation depends on the measured value, clinical context, and the guideline being used.
